Miscellaneous Agreements
Please read through each of these consents and check off the box indicating you have read the statement and agree to its terms and content.
Outside Program Agreements(Required)
Any arrangement/employment between me and a staff member of this center (i.e., babysitting), outside of the programs and services offered by this center, is an individual endeavor and a private matter, not connected to or sanctioned by The Nest Nursery School. The Nest shall remain harmless from any such arrangement.
Medication Dispensing(Required)
Before any medication is dispensed to my child, I will provide a written authorization, which includes: date, name of child, name of medication, prescription number, if any; dosage; date and time of day medication is to be given. Medicine will be in the original container with my child’s name marked on it.
Facility Entrance(Required)
My child will not be allowed to enter or leave the facility without being escorted by the parent(s), person authorized by parent(s), or facility personnel.
Keep Records Current(Required)
I acknowledge it is my responsibility to keep my child’s records current to reflect any significant changes as they occur (i.e., telephone numbers, work location, emergency contacts, child’s physician, child’s health status, updated infant feeding plans, updated immunization records, etc.).
Written Authorization(Required)
Care providers agree to obtain written authorization from me before my child participates in routine transportation, field trips, special activities away from the facility, and water-related activities.
Annual Calendar Received(Required)
I have received a copy of the calendar, which includes days and times that care is available and times when services will not be available (i.e. holidays and vacation).
Meal Acknowledgement(Required)
I understand that the school will provide lunch and a morning and afternoon snack.
Handbook Acknowledgement(Required)
I acknowledge that I have received a copy or have internet access to the 2025-2026 Nest Nursery School Family Handbook, which outlines the school policies in their entirety.
Participation Acknowledgement(Required)
While not a mandatory requirement, I understand that my participation in the life of the school is of utmost importance and agree to participate in classroom experiences, fundraisers and other social events, and parent meetings to the best of my ability.
Media Release(Required)
We regularly take photos of the children in conjunction with our classroom work at the center and for use within the center or on our website. We may also use photos during professional development initiatives in connection with Inspired Practices in Early Education and the North American Reggio Emilia Alliance. Please indicate that you authorize the use and reproduction of photographs of your child in conjunction with the program.
Occasionally we will post photos on social media sites. We try to reserve this practice for a specific message and/or special experiences. Please indicate that you authorize the use of photographs of your child on the school’s social media sites.
